What Information We May Collect

Advice Care may collect information directly from the participant, family members, guardians, carers, support coordinators, health professionals or other service providers where appropriate and with consent where required.

This information helps us understand the participant’s goals, needs, preferences, risks, communication style, health needs and support arrangements. Without the right information, it may be difficult to provide safe and suitable support.

Consent and Information Sharing

Consent means the participant agrees to information being collected, used or shared for a clear purpose. Advice Care will seek consent before sharing personal information with other people or services unless there is a legal, safety or emergency reason to share information.

With consent, Advice Care may share relevant information with support coordinators, family members, carers, guardians, allied health professionals, medical providers, other NDIS providers or emergency services. We aim to share only the information needed for the purpose.

Participants can ask questions before giving consent. They can also request changes to who information is shared with, unless information must be shared for safety, legal or regulatory reasons.

If a participant does not want certain information shared, they should tell Advice Care. We will respect the participant’s wishes where possible, while also considering safety, duty of care and legal obligations.
