Advice Care welcomes referrals from NDIS participants, family members, carers, guardians, support coordinators, allied health professionals, hospitals, schools and community organisations. Our referral process helps us understand the participant’s needs, goals, location, support requirements and NDIS plan details.

A clear referral helps our team respond faster and provide better information. When making a referral, please include the participant’s contact details, suburb, preferred services, NDIS funding category, support needs, risks, communication preferences and any urgent information we should know before contacting the participant or representative.

Advice Care provides NDIS supports across Melbourne and nearby suburbs including Mickleham, Craigieburn, Greenvale, Broadmeadows, Donnybrook, Beveridge, Wallan and surrounding areas. If the participant’s suburb is not listed, please still submit the enquiry and our team can discuss service availability.

The referral information should be accurate and provided with the participant’s consent, or the consent of an authorised representative where required. If you are unsure what to include, you can call our team and we can guide you through the process.

What Happens After a Referral?

After Advice Care receives a referral, our team will review the information and contact the participant, representative or referrer to discuss the support needs. We may ask for more details about the NDIS plan, goals, preferred times, location, risk information and service expectations.

If Advice Care can provide the requested support, we will discuss next steps. This may include an intake conversation, service agreement, support planning, worker matching, risk review and agreed service start date. We aim to make the process clear and respectful.

If a service is not available or not suitable, we will be honest and let the referrer know. Our goal is to provide safe and reliable support, so it is important that services match the participant’s needs and Advice Care’s capacity.

Support coordinators are encouraged to provide detailed information so the referral can be reviewed properly. This helps reduce delays and improves communication between all parties.
