After Advice Care receives a referral, our team will review the information provided. We may contact the support coordinator, participant, nominee or family representative to clarify service needs, location, funding and support risks.
If the referral appears suitable, Advice Care may arrange an intake discussion, request further documents, discuss service agreement requirements and begin support planning. This may include reviewing participant goals, routines, communication needs, health needs, risks and preferred support times.
If Advice Care cannot provide a requested service, we will aim to communicate this clearly. We believe it is important to only accept referrals where we can provide safe and suitable support.
