Advice Care welcomes referrals from NDIS participants, family members, carers, guardians, support coordinators, allied health professionals, hospitals, schools and community organisations.
A clear referral helps our team understand the participant’s needs, goals, location, support requirements, communication needs and NDIS plan details.

What Happens After a Referral?
After Advice Care receives a referral, our team will review the information and contact the participant, representative or referrer to discuss the support needs.
If Advice Care can provide the requested support, we will discuss next steps. This may include an intake conversation, service agreement, support planning, worker matching, risk review and agreed service start date.
If a service is not available or not suitable, we will be honest and let the referrer know. Our goal is to provide safe and reliable support that matches the participant’s needs.
